Cardholder Agreement
I verify that the information provided is correct and I assume financial responsibility for materials borrowed on any card issued from this application. The Cardholder agrees to pay replacement charges for materials lost or stolen. The cardholder further agrees to pay fees associated with the use of a collection agency used to collect fees and/or material that have not been returned.
No library record concerning an individual borrower shall be made available to any private party or government authority without a process order or subpoena and only after the approval of the library’s legal counsel. It is permissible to release information to the parents or legal guardians of a minor regarding that minor’s borrowing record.
All personal information contained in the patron files will be used only for valid library business. No personal information will be released to any party or used by the staff for any purpose other than library business.
E-mail addresses are used for account notifications and library news.
This card remains the property of the Wilmington Public Library of Clinton County and a replacement fee for lost cards may be charged.
Parental Permission for Minor Patron Access to Audiovisual Materials
Patrons under the age of 18 must have parental/legal guardian permission to checkout motion picture material. The Library's collection serves a wide diversity of customers and therefore contains material some feel is inappropriate. Parents should be aware that there are controversial, questionable, violent, and other materials in our audiovisual collections that are not appropriate for children.
By signing this form you the parent/legal guardian are giving the Wilmington Public Library of Clinton County permission to grant your child/ward access to any items in any of our circulating collections. Once access is given to a minor patron, the library staff will not restrict in any way that patron’s ability to check out any materials from our collections. This includes access to all motion picture material.
